Delaware's only Program of All-Inclusive Care for the Elderly celebrates
six year anniversary.
Wilmington, Del. -- Saint Francis LIFE celebrated its Sixth Anniversary
with an Open House on Thursday, February 28, 2019 at the LIFE Center on
the Wilmington Riverfront. Saint Francis LIFE will also announce the anticipated
opening of a second site in Newark, Delaware at the end of 2019.
"Saint Francis LIFE is so happy to reach this milestone of six years
of service to the elderly of New Castle County," said Amy Milligan,
Saint Francis LIFE Executive Director. "The philosophy of our program
is that it is better for frail, medically compromised seniors to remain
living in their homes in the community than in nursing homes. Our Wilmington
Riverfront site currently serves 259 seniors and we are in the process
of building a second full-service LIFE program on College Avenue near
the Pencader Business Park Center. We will be able to serve an additional
250 New Castle County seniors at this new site."

LIFE, which stands for Living Independently for Elders, is a PACE program,
or Program of All-inclusive Care for the Elderly. It is one of more than
100 PACE programs in the United States that offer this unique model of
care, and the only one in Delaware. In partnership with CMS and the state's
Division of Medicaid and Medical Assistance, the LIFE program serves individuals
who are 55 or older; live in the New Castle County service area; are certified
as needing a nursing home level of care, and can live safely at home with
the support of the LIFE team.
LIFE provides comprehensive care, including primary medical and nursing
care, physical and occupational therapy, nutrition services and social
work support at the center, and in the home. The program also arranges
specialty health care through its provider network of community specialists,
supports and collaborates with caregivers, as well as provides transportation
as needed. An Adult Day Program is also available for participants to
socialize and join in activities.
Saint Francis LIFE offers many advantages. Seniors stay in control of their
lives, remaining in the community and out of a nursing home as long as
possible. Its interdisciplinary team approach keeps seniors and their
families at the center of healthcare decisions. LIFE's care is complete,
preventative and ongoing. LIFE's compassionate care team provides
