St. Francis Home Care provides a comprehensive
program of personalized care for people who are able to
heal in the comfort of their own home. Offered through St.
Francis Hospital, our home care services include everything
from a program for new mothers and babies to complete assessments
of the elderly.
No two recoveries are alike
Our staff of experienced, specially trained
professionals works closely with physicians to develop the
best plan of treatment for each homebound patient's needs.
Our plan may be a continuation of a recovery program that
began during an inpatient hospital stay, or it can be developed
as the result of an outpatient visit. In either case, it
will provide an individualized, comfortable recovery plan
and all the quality caring people expect from St. Francis.
Will your insurance cover our program?
St. Francis Home Care program is certified
by Medicare and Medicaid, and recognized by insurance providers
who offer home care as a benefit. Please check with your
insurance provider to determine if your plan covers the
St. Francis Home Care Program. Members of our staff will
be happy to help you in any way we can.
Our professionals are hand picked to help
St. Francis Home Care provides professionals
who are ideally suited to meeting the needs of each homebound
patient. Working together with patients and physicians,
we perform an evaluation and develop an appropriate plan
of care. Our team keeps patients and their families informed
and involved each step of the way, so health care needs
are met—and understood.
Our registered nurses help patients and
families in all these ways:
- Monitoring medication
- Administering medication
- Changing dressings
- Providing catheter and ostomy care
- Obtaining blood samples for lab tests
- Patient education
- 24-hour on-call service
Our home health aides assist in these
everyday tasks:
- Attending to personal needs
- Attending to the comfort of the patient
- Providing assistance with exercise,
personal hygiene and general health maintenance under
the direction of a nurse or therapist
Our medical social workers go beyond clinical
care to provide:
- A healthy, supportive environment
for patients and families
- Counseling
- Assistance with insurance and financial
situations
- Referrals to community service organizations
Speech therapy
If speaking ability or swallowing reflexes
have been impaired, speech therapists work with the patient
to restore or develop speech patterns, improve verbal skills
and improve swallowing.
Occupational therapy
Occupational therapists help prepare patients
to perform daily activities such as dressing and eating;
relearn skills; and improve fine motor skills and physical
dexterity.
Physical therapy
Physical therapy can include:
- Training patients to use a walker,
cane or wheelchair
- Performing therapeutic exercises to
strengthen or increase movement
- Performing massage and ultrasound
treatment, and applying hot packs for pain and soreness
- Educating patients and families about
how to provide care during days the therapist does not
visit
Intravenous (IV) therapy program
Even when intravenous administration is
required, patients can still recover in the comfort of their
homes. IV treatments such as hydration, antibiotics, parenteral
nutrition and pain management can be administered safely
and comfortably at home by our team of infusion nurses.
And for further peace of mind, our professionals take time
to help patients and families understand all necessary equipment
as well as the proper administration of IVs.
Caring for new mothers and babies
The first few weeks of an infant's life
are filled with new challenges, but our Beginnings at Home
service is experienced at putting new families at ease.
Qualified nurses visit the home, assess the physical condition
of mother and child, educate new parents on infant health
and hygiene, and answer questions about baby's care and
mom's recovery. Periodic follow-up and a 24-hour help line
are just a few of the touches that help families adjust
to their tiny newcomer with confidence.
Extending our team
Other professionals, such as dietitians
and wound care nurses, may assist our health care team in
providing the best care for each patient. These specialists
will help make sure every aspect of our St. Francis Home
Care plan is ideally tailored to meet individual health
care needs. Medical supplies, equipment, personal care and
other necessities will be provided in accordance with each
patient's personal plan.
