Coverage is activated when enrolled in the health insurance plan.
The dental plan covers a wide range of dental services with an emphasis on preventative treatments, and is available through payroll deduction.
The vision program is available through payroll deduction. The benefits include eye exam, lenses, frames and contact lens.
Life Insurance and Accidental Death and Dismemberment (AD&D) Insurance is equal to the colleague's pay paid by the hospital. Additional supplemental coverage is available up to five times the colleague's pay.
Colleagues have the option to purchase dependent life insurance for their spouse (in $10,000 increments to a maximum of $100,000 or four times the colleague's salary, whichever is less) and children ($10,000 or $20,000).
Coverage is effective after one benefit year. Percentage of coverage and duration of coverage are based on years of service. Additional STD protection may be purchased.
Long Term Disability coverage of 60% of base pay is provided by the hospital. (Part Time colleagues are not covered under Long Term Disability (LTD) Insurance.)
Pre-tax dollars may be set aside to cover healthcare and dependent care expenditures. Maximum of $3,600 for healthcare and $5,000 for dependent care.
A colleague's paycheck may be directly deposited into banks and accounts of their choice.
Seven paid holidays each year including New Year's Day, Good Friday, Memorial Day, 4th of July, Labor Day, Thanksgiving and Christmas.
New hires are eligible for Paid Time Off after completing the three-month introductory period.
A contribution is credited to a retirement account for each year a colleague has completed 1,000 hours of service. Colleagues are vested after 5 years of service.
Colleagues have the opportunity to invest pre-tax dollars into a retirement account.
Colleagues are eligible for reimbursement for pre-approved job related courses through an accredited college or university for a grade of "C" or better. Full-time Graduate and Undergraduate: 100% reimbursement of up to maximum of $4,200 annually. Part-time Graduate and Undergraduate: 100% reimbursement of up to maximum of $2,100 annually.
A Colleague Assistance Program (EAP) provided by Human Management Services is available to all colleagues through a confidential 24-hour toll-free number.
